Toggle bold/italics with keyboard shortcuts

In the markdown editor, having text marked and pressing ctrl+b gives bold text but pressing ctrl+b again should toggle the bold styling off, not surround the text with further asterisks. ctrl+i for italic should of course also be toggleable.
Wasn't sure if this should be a separate request but they both are about behavior with bold/italic keyboard shortcuts:
It should be possible to, with no text selected, press ctrl+b and start writing bold text, just like in Word or similar programs.

Currently you need to select text before you can make it for example bold. However it would be nice that when you do
ctrl + b
it would insert **[cursor]**
so you can instantly start typing in bold.This of course with the other forms of formatting.
Discord currently has this in place (screenshot).
